Thirteen Months & 500 Steps

Last Tuesday my scans at Mayo Clinic still could detect no cancer in my body. That's called "complete remission" for a year and a month. Praise God! They told me that I am one of the longest surviving patients in this Revlimid clinical trial. One other man is two years out.

While there I climbed the stairs from the first to the tenth floor - twice! Total number about 500 steps. I do this most days at home, to build leg strength. One of the side effects of my medication is loss of muscle strength. So, I climb and I walk, and love it especially in this unusually cool, green summer of 2009 we've had.

However, where I plan to be with 100 other guests from early Sunday 9/13 through late Friday 9/18 is at Honey Rock Camp in Three Lakes, WI. Good Surgery Report

I ran outside today, without any pain, for the first time since my abdominal surgery two weeks ago today. The surgery was simple, and I recovered quickly. I left Central DuPage Hospital in just four days, instead of the 5 to 7 predicted.

While in the hospital, the best handle I had on how to be a man of God was to show cheer and gratitude to everyone who served me. The result was that I earned the name "The Happy Man."

I am grateful to God, to Dr. David Klem and to the good hospital staff. Dr. Klem explained to me after the surgery that the size of the passage from my small intestine to the large, before he started the surgery, would permit an object the size of a green pea to pass through it. When he finished enlarging it sideways into an expanded oval, he said you could slide a MacDonald's hamburger through it.

Good News Update on Dennis' Health Items

I am not going to have colon resection surgery on May 18 as planned. That has been postponed until late June or early July. I am doing well without much stomach pain or nausea.

The reason for the postponement was that last weekend I came down with bronchitis that put me to bed in a miserable and weakened condition. My pulmonologist gave me a "Z-pack" prescription of the anti-biotic Zithromax, which made a big difference in a few days. On Wednesday I'd have to say that God also lent me an extraordinary boost of energy and mental clarity as I presented my Vitality Therapy counseling training seminar for three hours on my feet, speaking without hoarse voice, to fifty Salvation Army counselors in Chicago. God overruled my illness! I loved that, and am in awe at what came over me. Friday I was back to being so fatigued that I got a transfusion of two units of red blood cells. I feel nearly back to normal energy today, but still like to sleep a lot.

My pulmonologist also discovered that my immunoglobulin levels are low. I will probably get 30 grams of intravenous gamma globulin some day next week. That has helped me get over respiratory infections in the past, but my last one was 1 ½ years ago.

I am scheduled for a day at Mayo Clinic Friday May 24, for my routine exams there. This will include PET/CT scans to see if there are any signs of cancer activity. If clear on that, I will begin Cycle #17 of Revlimid. This may need to be at a lowered dosage, because my bone marrow is showing signs of reduced ability to produce the blood cells I need, This is a disease condition known as "myelodysplasia" that often comes after prolonged lymphoma treatment. When the day comes that they detect cancer, or I get unacceptable side effects from Revlimid, I will be off that treatment and on to something new in the hope that it is more effective against my cancer, and less harmful to my body.

As to colon resection surgery, I would like to avoid it altogether, not just recover well from it. To that end I am probably going to explore some alternative medicine treatments, like acupuncture. I'm looking for something that has the ability to loosen the scar tissue constricting my intestines from the outside in. Please tell me any rumors you hear about in that direction.

Successful Surgery & Three Goodnesses

Last week I returned from successful surgery at Mayo Clinic to newly connect my small and large intestines. In addition I saw "the goodness of the Lord in the land of the living." - Ps. 27:13.

My generous brother, Howard, drove from Kansas City, to pick me up in Rochester, MN. From there we drove to my home, enjoying one of the easiest, flowing conversations I have ever had with any human being. Next morning he drove from Chicago back to Kansas City.
We pulled up to my front door in Glen Ellyn, IL on the dot of 10 pm Wed 2/18/09, to be greeted by an explosion of sunshine that was the stellar personality of my wife, Ruth.
The counseling sessions I have conducted since then have been marked by unusual mental clarity, not at all blunted by residual hospital medications or pain.

So, the goodness of the Lord in the land of the living. I didn't have to die and go to heaven to see it!

Two-Fold Dennis Health Update

Ruth and I spent Jan 25-28 at Mayo Clinic, Rochester, MN. The routine test for the presence of my cancer again showed no sign. Glory to God, and thanks to Mayo!

The second condition that took me there is going to soon send me back. Tests showed that stomach pains I've had for about a month probably come from a restriction where my small intestine joins my large intestine. Fixing it will require an abdominal incision on Feb. 9, probably a reconstruction of the junction, then a week or more of recovery in the hospital there at Mayo. This time is what it takes for our intestines to wake up again after a shock like surgery. But after that I should be able to eat a wider variety of foods, and not experience any more disabling pain.

As an investment in life after surgery, I have started building a presence on both Facebook and LinkedIn. It's so new that it doesn't show up yet. But It's fun to work on, and perhaps connect with you on it. I hope it will help me take my counseling business to the next level, especially with younger clients.

Clean As A Whistle

I just read the latest positive report from my Mayo Clinic cancer doctor. He said my recent scans were exceptionally clear, and showed no spots where lymphoma tumors had been one year ago. He exclaimed to himself and to me, "clean as a whistle!"

I take this as a huge item for which to thank God. But what about little items? I have learned recently about the name "Providence" which our nation's founders often used to honor God. It means that God is intimately involved in every detail of our lives for our good.

Better Yet On Revlimid

I am astonished! My latest visit to Mayo Clinic showed continued reduction of my lymphoma. I am now officially not just in "partial remission," but into "complete remission." That means there are probably still a small number of cancer cells in my body, but they are too few to detect. No patient who has responded this well, and continued taking my experimental drug, Revlimid, has yet had the cancer return. The only side effect I feel is reduced energy. We will slightly lower my dosage (a smaller pill each breakfast), to see if this gives me more stamina.

There is something special happening within me in recent months. God has given me this extraordinary relief from an "incurable" cancer. And my ongoing full-time counseling work has become more successful than ever before. The lesson here is that we learn wisdom from two major sources: suffering and gratitude.

Two More Good Months

My PET/CT scans at Mayo Clinic on 5-29-08 continue to look as good as they did on 4-10-08. That means a barely detectable smudge where there used to be a large, bright spot on the image. Conclusion: small volume, low activity, possibly even dead lymphoma after four months on the experimental drug, Revlimid.

I have been fortunate to have this drug work more effectively against the cancer in my body than it does in most other patients'. This makes me ponder the usual question: Why did God choose to send unusual favor my way? Or was this merely a biochemical coincidence that He allowed and that I happen to like? No matter. The issue for me is how best to be ardent with the delicious vitality entrusted to me.

Good Results From Two Months of Revlimid

In two months, the largest of several mantle cell lymphoma tumors in my chest shrunk from the size of a large egg, down to that of a dime. The dignified, reserved Dr. Thomas Witzig at Mayo Clinic exuded, "This is awesome! It doesn't get any better than this." He considered the amount of shrinkage my tumors showed (all little ones gone, and the big one just "debris,") as showing the activity of Revlimid at its best. PET scan's Standardized Uptake Value went from 13+ in Feb., to immeasurable in April. He plans to use the two PET/CT scans of mine in the major research paper he will author on the 211 relapsed MCL patients who took Revlimid as a single agent. I assume this paper will be a major basis on which the FDA makes its decision whether or not to approve the use of Revlimid for treating relapsed MCL.

In my case Revlimid may have been helped a bit by my use of Xango, the tropical juice mangosteen, during the last two weeks before my April PET scan. This juice's action is said to be anti-inflammatory more than anti-oxidant.

I am now taking my third 21-day supply of Revlimid.. My next scan will be in two months, meaning early June. I am finding that the disruptive effects of Revlimid on my digestive system seem to be cumulative. It was more gentle in rounds 1 & 2.

Mayo Clinic Revlimid Treatment

Tomorrow I will drive the 336 miles in seven hours to Mayo Clinic in Rochester, MN. Quick visit with the good and famous Dr. Witzig on Tuesday, then return home with my second 21-day supply of cancer-fighting tablets to take once a day at home. I started into this clinical trial of the drug Revlimid a month ago, and it's going pretty well so far. Just a few minor side-effects, like some hoarseness in my voice, and difficulty pronouncing some word combinations. like "the Lord." Too bad; that's one I like to say well!

Last month Mayo found that my cancer is showing up in a couple of places where it has not before. The total amount is still small, but it grows more aggressively than it has in the past, when I am off treatment for even one month. This means that my future treatments will have to be more rugged, probably not leave me looking or feeling as good as previous ones have. In April I will get my next scans that show what effect the Revlimid is having. If my mantle cell lymphoma grows, we will have to decide what my fourth treatment for this tenacious disease will be. Several effective treatment options are open to me.

My strategy is to keep putting together novel treatments that give me a year at a time, without too much depletion of my wellness. Then, five years from now, some researchers are saying they will probably have a cure, in the form of a cocktail of about five ingredients, each of which brilliantly blocks a different pathway of survival of the cancer cells, so that they give up and die. Revlimid is likely to be one ingredient in the cocktail, blocking the formation of the tiny blood vessels that tumors need to supply nutrients to them.

Thinking life-and-death thoughts like this tuned my eyes to notice the exquisite sparkle of snowflakes on the ground as I was walking the Prairie Path the other day. Sure, the winter has been long, and the amount and persistence of the snow tedious. But to receive the gift of new eyes, to see the friendly glint of the warming sun off the facets of the snow crystals - ah, that is one of the sweet legacies of having cancer, and a good God who's in it with me.

Cancer Update: Approved To Continue With Successful Treatment

I have gotten approval from the research team at Mayo Clinic of Rochester, MN, to continue the treatment I have liked for this past year with CCI-779 (mTOR inhibitor, temsirolimus, Torisel). After that one-year clinical trial, I am in "stable condition", the single 3-cm lymph node in my chest not having grown, and my quality of life hardly having been hurt at all - just a little loss of stamina. Other graduates like me evidently are asking to be allowed to continue this treatment until disease progression. So, Mayo has revised its clinical trial protocol to permit this. You might view it as Stage IV clinical trial data gathering about effectiveness and toxicity of long-term exposure to the treatment combination with relapsed MCL patients. The question of how this will be paid for now outside a clinical trial remains to be seen. I like the motto in life, "Don't ask permission, ask forgiveness." We will run the insurance claims through as usual and see if my company continues to be as marvelously helpful and uncomplicated as they have been. If they balk at paying for a treatment not yet approved by the FDA, I will plead my case that (a) Torisel is an approved drug, and (b) we know it's an effective off-label treatment for me because it has stopped my tumor from growing for the past twelve months.

Update on Dennis Gibson's Good Health

My oncologist walked into my examination room today, smiled big, and shook my hand. He told me that my latest scans showed no growth of my cancer in the past three months. So, it has been dormant since I started into this clinical trial with Rituxan and Torisel nine months ago. And I have had so few and mild side effects that I cannot tell I am getting cancer treatment. Now that is a marvelous kind of breakthrough!

Now I face what to do after I complete the twelve months of this trial. I prefer to be allowed to continue receiving these two medications under a category called "compassionate use," or "off-label treatment." That means getting special permission to use these drugs that are approved by the FDA for treatment of other conditions, but not yet for use on my mantle cell lymphoma. So, here is a treatment shown to be keeping me from dying, but which the FDA may take years to decide is OK for me to use. I am among thousands of cancer patients who face denial of treatments that can save their lives, simply because of an unnecessarily slow, bureaucratic process operating within the FDA.

The CCI-779 Drug I've Been Getting Now Has a New Name -- Torisel

The link below tells about the FDA's approval of the drug as "Torisel" on May 30. It was approved for use against renal cell carcinoma. My mantle cell lymphoma is the second disease in which the manufacturer is seeking FDA approval. One bright avenue that the May 30 action opens for me is "off-label use." That means that its approval for one use allows doctors to prescribe it for other uses, since its safety in human beings is now established. So, I may be able to have it prescribed for my treatment extending beyond the end of my clinical trial on November 15. Some interesting technical description of the drug's novel mode of action.

Update on Dennis Gibson's Health

Scans taken last week show no growth of my cancer. No shrinkage either. So my condition is officially "stable." I was therefore able to receive treatment today with CCI-779. This is week #2 of Cycle #7, out of twelve, four-week cycles. My blood counts are improved. Platelets holding at 80,000. My energy level is very satisfying to me. My mental alertness is good. I would be content to live many more decades on this current schedule. But when the clinical trial is over, it will be up to me to work hard to persuade the drug company and the government and my insurance company to allow me to continue treatment with this drug that is not yet approved. "Compassionate use" is the term for this kind of exception, when someone else continues to pay for a not-yet-approved treatment, known to work for me, but beyond my means to afford. If I cannot succeed in that and my cancer starts growing, say next year at this time, I will return to actively searching for another form of treatment showing promise. One I've heard of is a transplant in Seattle. Another would be PR-171 in New York or North Carolina. It reminds me of a soldier home after three tours of duty in Iraq, now told that the Army is sending him there again. But I am God's and He may do with me as He pleases, and I am ardent to be pleasing to Him in how cheerfully and diligently I carry out His assignments.

Update on Dennis Gibson's Health

"Dennis, you've turned the corner!" My wife spoke as a prophetess about my progress over cancer. A chorus of consensus shouted within me, "I'm sure that's true." It was like the writer, Paul, expresses in Romans 8, that the Spirit of God bears witness with our inner spirit - consensus.

This fresh vitality came a few days after an infusion of packed red blood cells lifted me out of weakness, dizziness, and discouragement caused by low hemoglobin.

My experimental treatment has been tweaked to a low dosage of 10 mg CCI-779 every other Thursday. That level keeps my platelet count well above the 50,000 minimum necessary to qualify for treatments. Yesterday I was treated at 65,000! Next week I'm due for CT scans to see how well this treatment is continuing to shrink this "incurable" lymphoma in my chest. I might be helping to prove the usefulness of a treatment that will be repeatable over the years. So, we can manage the condition like other chronic diseases, like diabetes, with minimal reduction in quality or length of life.

This June has been the greenest, most luxuriant I have ever seen. My soul fills with life and worship! I am eager to increase the number of clients I serve in my office counseling practice and cancer coaching by telephone.

Missed Today's CCI-779 Treatment

My platelets were 43,000 -- not up to 50,000 minimum necessary to qualify for today's lymphoma treatment. Implication for future treatments: I am now in the lowest category of treatment intensity. That means I will henceforth get only 10 mg every other week. This compares to original max 25 mg every week. My oncologist reassured me today that they do everything they can to keep patients in the study, instead of just dropping me next time my platelets are below 50,000 on a scheduled treatment day.

I did get the other chemical that is part of this combination treatment, Rituxan, which has been a good lymphoma killer for me in the past just by itself. My present study is based on the expectation that the two chemicals together will show greater anti-cancer power than either alone. My schedule calls for a Rituxan infusion the first Thursday of every odd numbered month. I'm doing OK, but disappointed.

This Thursday's Treatment Went Well

My platelets were up to 61,000, so I qualified for my 10 mg dose of CCI-779. This puts me 1/3 of the way through my full treatment in this clinical trial, having ended Cycle 4 of 12 -- each cycle taking one month of once per week treatments. The effects are finally catching up with me. Until now I have not been able to tell that I am receiving any cancer treatment at all. But now I am drowsy and fatigued much of the time, and sleeping 2-3 hours twice during daylight hours.I consider this a small price to pay for what I think is turning out to be a highly effective treatment.

We had three families come and look at our house as potential buyers. All three loved it for different reasons. One was a substance abuse counselor who would like to use our place as a home and office, like we have done. The second was a three-generation family in which the grandma loved the office portion as an in-law apartment for her. The third family was one with six kids, the older two loving the office as two bedrooms, with bathroom, and a TV room. The younger kids liked the crawl space under the office as a fort hide-away.

Then Thursday we had an open house for realtors, of which seventeen came through and liked it. The condo unit we want to move into is empty now and ready for us to start decorating as soon as we close on our house. The sooner the better for saving money.

Successful Treatment Today!

My platelets were 57,000, qualifying me for a 10-mg infusion of CCI-779, to keep making my cancer smaller and smaller. I was encouraged by a report last week from a friend, that he knows someone who was treated with this drug, with splendid results. I continue to be amazed that I feel no side effects from treatment. I think this drug, in combination with others, like my Rituxan, has a bright future for several varieties of cancer.

Latest Lymphoma Treatment News

Yesterday I did not qualify for my weekly treatment in the clinical trial with CCI-779. My platelets were just barely below the 50,000 minimum necessary, at 49,000. This despite my use of sesame oil and agrimonia Chinese herb that has raised my platelets in the past month. When I talked to the tech man at the company that supplies the herb, he said it works temporarily, like for about a month. So, I'm going to give it a rest and see if it resumes its ability to raise my platelets in a week or two.

I was initially disappointed to miss a treatment, thinking that I need one each week so as not to die from lymphoma. However, I am comforted by another factor related to the design of this clinical trial. It specified 25 mg of CCI-779 per week as initial dosage. The first time a patient misses due to platelets below 50,000, future doses are lowered to 20. Missing one week at 20, your next dose is 15. Missing one week at 15, your next one is 10 mg. Failing to have a 50,000 platelet count for one week at 10 mg means you drop to the minimum treatment category: 10 mg every other week. Missing several of those means you are dropped from the study. I just failed at 15, so my next treatment will be at 10 mg. I think my body will probably recover from 49,000 this week to above 50,000 next week, since I am not being hit by the chemical this week.

The factor that comforts me is the recognition that the researchers have reason to believe that CCI-779 might retain its anti-cancer effectiveness at lower doses than originally thought -- like 10 rather than 25. In their original work, they thought 250 mg was the right dose. Then they found that 25 worked just as well against cancer, with much less hit to the platelets. So, I get to be one of those experimental subjects who explores the low limits. The lower the dosage, the less toxicity in terms of reducing platelet counts. I have the chance to show that low doses can both kill cancer and preserve healthy platelet counts. I like lending my body to the advancement of human knowledge, while at the same time using a treatment that may do better against my cancer than methods that have been used in years past. I think this attitude fits the Christian concept of the stewardship we are to exercise with resources God entrusts to our management. Cancer & House Progress Update

My platelets have been high enough for me to get my CCI-779 treatments the past two weeks. I have completed the four weekly treatments of Cycle #3 . It may take as long as 12 cycles -- one year. But my malignant lymph node has shrunk from 3 cm to 2 cm. We are beating this thing!

Our house is in mint condition for sale. Realtors have come through looking at it, and being impressed, but no shoppers yet. Our listing realtor plans an Open House for Sunday April 15, with lunch served. We have a contract on the condo unit we want, pending sale of our house. In our new location we will rent office space just 1/2 block away, thus continuing our counseling practice.

I am teaching a series about the miracle stories of Jesus. Those who benefited from His miracles knew they had come to the limit of their powers to influence events. They looked to Him for supernatural intervention. Now with our home sale, we would love it if we could conclude all the selling and moving by mid-May to save money. So, we ask the Miracle Worker to direct a buyer to us, and have us agree on a fair price, during April. But we follow His example, which Christians will commemorate this coming Thursday, when He acknowledged that God is not obligated to serve our wishes. Therefore, "Not our will but Your will be done, Good Father."

Success In Raising My Platelets

I was able to proceed with my cancer treatment today using CCI-779. I had missed two weeks because my platelet count was just below the allowable minimum of 50,000. My medical advisors said there is no medicine to raise platelets, like there are injections to raise red or white blood cells. The only thing I could do is wait for my body's bone marrow to spontaneously rebuild my platelet count. In one week it raised from 47,000 to 49,000. So, starting a week ago, I began an experiment to see if I could raise my platelets at a faster rate than that.

I learned from an excellent cancer nutritional expert, Dr. Kim Dalzell http://www.cancercenter.com/midwestern-hospital/physicians/kim-dalzell.cfm that some patients have been able to raise their platelets by adding sesame oil to their diets. So, for the past week, I have taken 1-2 oz sesame oil morning and night.

Then I Googled something like "raise platelets," or "increase platelet count" and found http://www.itp-helper.com. I called Kim Dalzell about the ITP Chinese herb, agrimonia, to see if it might negatively interact with anything else I am taking. It was OK, so I ordered ITP Booster and started taking six per day.

I reasoned that any platelet level today higher than 60,000 would be hard for my body to achieve unaided, Therefore, any number over 60,000 would tell me that my experiment had found a way that patients can raise their own platelet count. My number today was 82,000. I want this information to be known widely by cancer patients whose treatments have to be restricted by lowered platelet counts.

Cancer Treatment Update - Stopped Growing, and Not Yet Shrinking. Stable.

I have finished Cycle #1 of five in a clinical trial with CCI-779 plus Rituxan. Four once-weekly treatments per cycle. Few side effects. Only one for me is diarrhea, which has forced me to a mild diet, mostly white rice, chicken broth, applesauce, and carrot juice. I am pleased.

Into My Second Treatment

After four years of my mantle cell lymphoma being undetectable, it was detected last month, in one lymph node, slow growing, in small amount. It should respond well to treatment that I began Thursday in a clinical trial using Temsirolimus with Rituxan. I feel no side effects either of the cancer or the treatment. My energy is great. I am counseling full time, more effectively than ever. I am at my prime.

This treatment represents the new targeted therapies flooding out of research labs. It’s as if the army developed a secret weapon that could disable an entire enemy army by identifying and eliminating one single enemy soldier – the one who pushes the button to wake all the others up.

I will go for four hours, one day per week for five months, to a hospital in nearby Aurora. It is part of a network coordinated by Mayo Clinic. I am patient #29. No overnight hospital inpatient stays.

We are also downsizing our living quarters. So, send buyers our way who could use a specialty house that can serve either as a home and office, or home with in-law apartment, or five-bedroom & four-bathroom home. We plan to move into a condo nearby in Wheaton, and use rented counseling office space.

So, please pray for the success of this treatment in again making my cancer undetectable, without troublesome side effects. And for a good sale of our home soon. And for continued positive impact for the kingdom of God by our counseling and teaching activities.

Close Call

At 1:35 pm yesterday, I sped my car across our busy road toward my driveway. A routine maneuver. I first checked to my left and saw a van beginning a right turn just to the left of me. Reassured that there was no more traffic coming from my left, I looked to the right. Finding that direction clear I gunned my car from the stop sign across toward my driveway. A car that was hidden from my view in the inside lane behind the van on my left nearly smashed into my driver’s side door at 40 mph. I swerved hard right. He swerved hard left. No crash. No damage to either of us. Not even any slowdown in either of our journeys. In a second it was all over and we continued our lives as planned.

How fragile our plans! It could so easily have been a totally different story, that changed my life forever. I could have been lying paralyzed on my back in the street, looking up into the faces of paramedics. Or looking down at the dead body of the other driver. I have focused on the threat of cancer to my life over the past four years. Now the near crash was a wake-up call to slow me down from rushing, to renew my vigilance when driving, to live with that deliberate sweetness that goes with continual appreciation of moments.

The cancer that I thought would forever end my short-term mission trips actually became the vehicle for eight days of ministry in Quito in January. Of twelve public presentations I made, eight were to audiences of cancer patients and their caregivers. God delights to bring good out of evil. I now continue growing in stamina and overall health with no detectable cancer. Lesson: continue my health diligence, yes. But remember my ordinary mortality and guard it with unhurried alertness.
